ACCMA Members Health Insurance Plan Information

The ACCMA and PacifiCare are pleased to announce the NEW HSA PPO 3500 plan into our member portfolio for 2008 open enrollment. As always the plans are available to all ACCMA members and family on a guaranteed issued basis. Please submit your ACCMA PacifiCare application to JC Lewis Insurance Services in November or December to process your enrollment promptly. The application and coverage change request forms are listed below for you to print out, fill out and send back to be approved.

ACCMA PacifiCare Healthcare Plan Highlights

When to apply for ACCMA PacifiCare healthcare plans?
The ACCMA open enrollment for PacifiCare healthcare plans begin November 1, 2007 and runs thru the end of December 2007. The effective starting dates can be December 1st, 2007 or January 1st, 2008. Existing enrollee's may submit plan change requests for December 1st, 2007 effective date only - forms must be received before December 28th 2007.
